Please use this T-shirt order form ONLY if you are not attending the conference, but would like T-shirts.  If you are attending the conference please put your order on the registration form on pages 5-8.  T-Shirts are $10 each.  Shipping is $5.00 for every 3 shirts ordered.  ORDERS MUST BE RECEIVED BY JULY 1, 2015.
